SUMMER CAMP - IGNITE | FUEL | MOMENTUM FORMS
* Registration Form - (or Register Online)

The following forms are required and some need to be turned in at least 1 month before camp
* Camper Preparation Packet (includes all the following)
          * Medical Form
          * Medication Card
          * Packing List & General Info
          * Camper Release Form
          * Activity Waiver Form
          * Horsemanship Experience Form (for Horsemanship Campers Only)
          * OutTrip Request Form (for Momentum Campers Only)

*Scholarship Application Form (Registration Form also required)

 

DAY CAMP FORMS
* Day Camp Registration Form - (or Register Online)

The following forms are required and some need to be turned in at least 1 month before camp
* Day Camp Preparation Packet (includes all the following)
          * General Info & What to Bring
          * Medical Form
          * Camper Release Form
          * Day Camp Options: Lunch, Early Drop Off & Late Pick Up
          * Medication Card

 

FRIENDSHIP CAMP FORMS
* Friendship Camp Application Form (Online Registration NOT available)

The following forms are not required, and will not be accepted, until after May 1
* Friendship Camp Preparation Packet (includes all the following)
          * Packing List & General Info
          * Friendship Camp Medical Form
          * Camper Release Form

*Scholarship Application Form (Application Form also required)
